To register as a patient at West Park Surgery please come to the surgery to pick up a registration pack.
Before returning the Registration Forms:
- Ensure all parts of the forms are completed – otherwise you will not be accepted
- Bring Photo ID (Drivers Licence, Passport etc.) – This is to verify who you are
